Emergencies

If you are calling regarding an emergency, DO NOT LEAVE A MESSAGE THROUGH OUR WEBSITE SINCE IT MAY NOT BE CHECKED UNTIL THE FOLLOWING BUSINESS DAY.

If you are experiencing a life-threatening emergency, call 9-1-1.

If you have an urgent question or think you may need to be seen emergently, call (312)654-1166. The receptionist may ask the nature of the emergency so she knows if she needs to interrupt a physician who may be seeing a patient, or if it is something that can wait a short time. She will ask your name and date of birth so your medical record can be pulled. If the Nurse Practitioner is available, she will speak with you to find out the nature of the emergency.

If the automated system is activated, press the "if this is an emergency" option. You will be connected to a live person who will direct your call to the appropriate person. If you get a voicemail, leave a message. Someone should return the call within 30 minutes. If they do not, please call again.

After office hours, emergency calls will be directed to the physician on call.

Call (312)654-1166 and press the emergency option. The physician on call will not fill routine prescriptions and will not have access to test results. We encourage you to call during regular office hours for any routine inquiries.

If you need to be seen and it is after hours or inappropriate to see you in the office, we will direct you to the emergency department of Northwestern Memorial Hospital located at 251 E. Huron.

The Emergency Department is located in the Feinberg Pavilion, not Prentice Women’s Hospital.

From the Kennedy Expressway (I-90)

  1. Exit on Ohio.
  2. Go east on Ohio to Michigan Avenue. Turn left on Michigan.
  3. Go north on Michigan to Erie Street.
  4. Turn right on Erie and go East to 441 E. Erie for parking (b) or take Erie to inner Lake Shore Drive and turn right for the W Hotel Parking at 644 North Lake Shore Drive.
  5. To park in the hospital parking lot, take Michigan north to Superior, turn right and enter garage on Superior. Walk one block south to Huron and east to 680 North Lake Shore Drive. 
From Northbound Lake Shore Drive
  1. Take Lake Shore Drive north to the Grand-Illinois exit.
  2. Stay right and follow Grand Exit. Take Grand to McClurg Court.
  3. Turn right on McClurg Court and go north to Erie Street.
  4. Turn right on Erie and go east to 441 E. Erie for parking (b) or take Erie to inner Lake Shore Drive and turn right for the W Hotel Parking at 644 North Lake Shore Drive (c) or take McClurg Court to Huron.
  5. Turn left on Huron and go west to hospital parking.
From Southbound Lake Shore Drive
  1. Take Lake Shore Drive south to the Michigan Avenue exit.
  2. Take Michigan south to Erie Street.
  3. Turn left on Erie to go east to 441 East Erie for parking (b) or take Erie to inner Lake Shore Drive and turn right for the W Hotel Parking at 644 North Lake Shore Drive (c) or take Michigan to Superior, turn left on Superior and go east to the hospital parking garage.
  4. Walk one block south to Huron and east to 680 North Lake Shore Drive.
Parking
Here are the options for parking structures and the corresponding validation rates. Please obtain a validation sticker or stamp at the front desk upon check-out. Rates are subject to change.
  • 441 East Erie: 3 hours for $12.50
  • W Hotel Parking, 644 North Lake Shore Drive: 3 hours for $10
  • Northwestern Memorial Parking, corner of Huron/St. Clair: 4 hours for $10.00
  • Please note: Parking is available in our building at 680 N. Lake Shore Drive; however, reduced-price validation is not available for this location.  Full-rate parking fees will apply.
Medical Records

We are happy to provide a copy of a particular test result or report to you or another physician.

All medical records are the property of Gynecologic Specialists of Northwestern. 

If you require a copy of your entire medical record for insurance purposes, transfer of medical care, or for your personal use, there will be a fee to cover copying and administrative costs.

If you need records for a physician we have referred you to (obstetric patients, fertility specialist, oncologist, etc.), we will copy relevant records at no charge.

If you are moving, changing practices, or have a new internist, you generally do not need a copy of your entire medical record.  New physicians generally only require relevant test results and operative reports. We can provide the information you need, and in the event you need more information at another time, you can always request it.

We must have a records release form prior to initiating the copying process. Please allow five working days for duplication of an entire record. Medical records may not be faxed according to Federal Privacy Laws. If you have any questions, please reference our HIPAA policy.

Cancellation/No-Show Policy

We understand that there are times you are unable to make a scheduled appointment. Please cancel 24 hours prior to your appointment time to avoid a cancellation fee. If you are a new patient, you will not be able to reschedule unless you submit a non-refundable deposit of $100, which will be applied to your visit or refunded after insurance payment.

If you arrive late for your appointment, we will do our best to accommodate you, but please understand that that is not always possible, particularly if you have a procedure or ultrasound scheduled. If we cannot accommodate you, you will be offered another appointment or, if available, the option to see another available physician or provider.

Please make sure we have your contact information, such as a cell phone number, so that we can contact you if we are running late or your doctor needs to leave unexpectedly.
